Innovations in Care: Here 24/7 is Now Accepting E-Referrals

The Canadian Mental Health Association Waterloo Wellington is the lead agency for Here 24/7, the single access point for addictions, mental health, and crisis services provided by 11 agencies across Waterloo Wellington.

Here 24/7 is now accepting e-referrals, replacing traditional fax-and-paper systems and thereby creating a more efficient way for primary care doctors to connect patients to Here 24/7 and community mental health and addiction providers. The benefits of e-referrals are many, including: receiving live updates on the status of a referral and being able to correspond with Here 24/7 service coordinators during the process to answer any questions.

Some of the services that require referrals through Here 24/7 include:

• Dialectical Behaviour Therapy (DBT)

• Outpatient Eating Disorders Program

• Community and Residential Addiction Programs

• Concurrent Supportive Housing

• Adult Intensive Teams including FACT and ACT

As a core member of the Guelph Wellington Ontario Health Team, CMHA WW, has supported strategic priorities such as increased use of electronic referrals (e-referrals) to replace faxed referrals in Guelph Wellington. E-Refferals will help residents have better access to mental health and addictions services when and where patients need it most.
