Compliment and/or Feedback
If you and/or your family, loved one has had a positive experience with Canadian Mental Health Association Waterloo Wellington (CMHA WW), that we can learn from please let us know. Your input will help us to keep up the good work we do that is of value to you.
Complaint
On the other hand, if you and/or your family/loved one finds your experience with CMHA WW services unacceptable or unsatisfactory, please follow the process as set out below. We welcome your input at all times and hopefully we can address your concern either as it happens and/or shortly after.
Confidentiality of Feedback/Complaint
Any information we receive from you and/or from anyone advocating on your behalf will remain confidential and be communicated only to those people who need to know. Specific personal health information will be anonymized where appropriate. CMHA WW takes your privacy seriously and must comply with Ontario Privacy laws.
Compliments and Feedback Process/Procedure
Compliments and feedback may be received in the following methods:
- Complete a Client or Caregiver Experience Survey:
- If you are a client, complete the Client Survey
- If you are a caregiver/family member of a client, complete the Caregiver Survey
- Complete a paper survey at any CMHA WW office locations
- Email: feedback@cmhaww.ca
- Call: 1-844-CMHA-WW3 (2642-993) ext. 1800 to complete a survey over the phone

Clients and families may view the CMHA WW Complaints, Compliments and Feedback policy here. Complaints may be received in the following methods:
- Complete a paper survey at any CMHA WW office locations
- Online by completing the complaint form here.
