About Us
Special Services At Home Coordinators work with families of children with a Developmental Disability, and with families of children with a Physical Disability. Family Support Options has an ongoing commitment to provide the level of support required by families to negotiate the system successfully. Coordinators will assist families to utilize funding via both options available to them, Independent Model and Employee Model.

Email and Texting Policy/Disclaimer
The Canadian Mental Health Association of Waterloo Wellington, in keeping with electronic forms of communication, has begun to look at the use of email and texting. What this means is that we recognize many of our families, Individuals, and staff find this an effective way to stay in touch and communicate with each other. Although these forms of communication are not considered secure, we as an agency feel that for the management and scheduling of appointments it can be appropriate. We are being very clear in our message that it should not be used for sharing personal information, problem solving, consultation, and crisis situations. If you have any questions in regards to the use of email or texting please do not hesitate to connect with your Family Support Options Coordinator.

Transition to Adult Services
To access supports for the program participant as an adult an application must be made to Developmental Services Ontario (DSO). DSO’s role is to help adults apply for Ministry funded adult developmental services.
When the program participant turns 16, contact with the Developmental Services Ontario office (DSO) should be initiated. Although not eligible for adult services till age 18, DSO can accept diagnosis documentation at age 16. This is required to determine whether the individual is eligible for adult developmental services. Please be aware that although they have been eligible for services as a youth, this does not guarantee their eligibility as an adult. The DSO has established new eligibility criteria for those 18 years of age and older. Once the DSO receives your information they will review it in an attempt to confirm eligibility and contact you regarding what else is needed. A psychological assessment is required to confirm eligibility for services and supports.
