Intensive Geriatric Service Worker Program Wins The Village of Wentworth Heights Team Award

The Canadian Mental Health Association Waterloo Wellington Dufferin Intensive Geriatric Service Worker team was recently recognized at the 13th Annual Service Awards for Geriatric Excellence (SAGE). SAGE is a unique program that celebrates the outstanding contributions of individuals and organizations committed to providing the highest quality of care to older adults in our communities to ensure they can age with optimal health, independence and dignity. The awards were presented at a gala dinner on Friday May 13, 2016 at the Crowne Plaza Fallsview, Niagara Falls, Ontario. 52 nominations were received from seven regions across Southern Ontario (Hamilton, Niagara, Haldimand-Norfolk, Waterloo, Wellington-Dufferin and Halton).

The Intensive Geriatric Service Worker team won the Village of Wentworth Heights Team Award. The IGSW program is very innovative and demonstrates outstanding evidence base best practices. The individual worker is passionate and strives to meet the needs of the very vulnerable senior who is often at risk medically and/or from a mental health perspective.

The team receives referrals from the Geriatric Emergency Management (GEM) Nurses in the Emergency Departments of the Hospitals; Advanced Practice Nurses in Geriatric Medicine; Specialized Geriatric Services – Geriatric Medicine or Geriatric Mental Health; and Primary Care Geriatric Specialty Nurses. This team has been referred to as ‘angels’ who arrive at the right time and right place to assist the senior with achieving their goals, which often is to remain in the community; out of hospital or to prevent premature transitions to a Long Term Care Home.
